This memo provides information on the use of an alternative anticoagulant in the event of a shortage of phenindione tablets. Its content  is largely based on consensus opinion from practitioners based at Guy’s and St Thomas’ NHS Foundation Trust and outlined in the PGD cited in the memo. The dose conversion and monitoring guidance is not based on published evidence and users should bear this in mind in deciding whether to base their policy on this document.
