Product discontinuations and shortages have been a problem for pharmacy managers and other healthcare providers for many years. They can arise for many reasons such as unavailability of raw materials, manufacturing difficulties or lack of commercial viability and they can potentially impact upon the quality, timeliness and cost of patient care. When shortages or discontinuations occur, those responsible for sourcing and providing medicinal products need fast access to information regarding the reasons behind the problem, when products will be available, alternative sources and alternative therapies. The memos published here have been written by medicines information pharmacists in response to current supply problems and are intended to address these information needs. They are presented in a format that can be used as a memo to alert other healthcare professionals of the issue and direct them in managing the problem. It is intended that the memos be tailored by individual centres to fit local needs e.g. local Trust logo can be added and content can be modified or summarised according to the needs of the recipients. Please note, the original authors name and place of work will be included on the memo, but please remember to add the name of any person who modifies the document plus the name of a person to be contacted at the base hospital in the event of any queries, to avoid the problem of queries being inappropriately directed towards the original author or their centre.  Click here to see the PSNC's list of Medicines that Pharmacies have reported having problems obtaining.
