In this review of chronic hepatitis B therapy, the authors provide an overview of the mechanisms that influence the barrier to resistance, including genetic barrier, cross-resistance, drug potency, patient adherence, pharmacological barrier, viral fitness, and drug site of action. They also highlight the importance of selecting a first-line therapy with a high barrier to resistance, and review alternative strategies for the prevention of resistance in countries with limited health-care resources.
